Should my mom use her name when sighning me up for a schooL!!!HelP plEasE?

I want to go to a better school then i did last year in the 7th grade. But the problem is i don't live in the area to go to the school. Two of my cousins go to the school and their parents said we can use their address. But my mom doesn't know if she should her name down as my parent or my cousins mom. She looked at the school site and it said you must have proof of address. It said you can use an old water bil or gas bill to show proof. But its go in to have my cousins mom name on it. Me and my mom are so confused.
